Minimum Referral Criteria

Category 1
  • Documented cataract with documented significant impact on activities of daily living (ADL) and BCVA worse than 6/36 in each eye
Category 2
  • Documented cataract with significant impact on ADL and:
    • BCVA worse than 6/36 in one eye or
    • BCVA worse than 6/12 in each eye
Category 3
  • Documented cataract with significant impact on ADL and BCVA worse than 6/12 in either eye

If your patient doesn't meet the minimum referral criteria

  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service.

Essential Referral Information

  • Best Corrected Visual Acuity (BCVA) (vision with most recent distance spectacles) with refraction in the last 12 months
  • Whether first or second eye
  • Symptoms and duration of problem

If a specific test result cannot be obtained due to access, financial, religious, cultural or consent reasons a clinical override may be requested. This reason must be clearly articulated in the body of the referral.

Additional Referral Information

  • Private ophthalmologist or optometrist report including VA, refraction and impact of symptoms (highly desirable)
  • Professional drivers with specific VA requirements for employment
